“Rethink the Stink”: Cape Town’s Sewage Crisis Puts Public Health and Ecosystems at Risk

Cape Town’s iconic beaches and waterways—beloved for their beauty, tourism, and biodiversity—are in the spotlight for all the wrong reasons. While the waves sparkle and Table Mountain stands tall in the background, an invisible threat lurks in the water: sewage pollution.

A Lagoon in Crisis

Take the Milnerton Lagoon, for instance. From afar, it’s postcard-perfect. But come closer, and the smell of sewage is unmistakable. Residents like Caroline Marx, a long-time campaigner and member of citizen science group Rethink the Stink, describe waking up at night from the stench and witnessing years of ecological collapse.

“You can’t believe the smell when there’s a major spill,” she says. “And every time the lagoon starts to recover, another one hits.”

Citizen Science vs. City Hall

Rethink the Stink isn’t just making noise—they’re collecting data. The group recently released a report, Project Blue, showing spikes of E. coli and enterococci—bacteria that indicate faecal contamination—at some of Cape Town’s most popular “Blue Flag” beaches, including Clifton Fourth and Camps Bay.

But city officials have pushed back, questioning the labs’ testing credentials and defending their own monitoring programs. Deputy Mayor Eddie Andrews insists Cape Town has “the most transparent and comprehensive water monitoring in South Africa.”

Still, many remain unconvinced.


The Bigger Problem: Marine Outfalls

Even beyond the lagoon, concerns are mounting about Cape Town’s three marine outfalls, which release 28 million litres of partially treated wastewater into the sea every day. That’s roughly 11 Olympic-sized swimming pools—pumped into the ocean just a few hundred metres from shore.

Researchers like Prof. Leslie Petrik (UWC) and epidemiologist Jo Barnes have traced the impacts of this pollution to marine organisms such as mussels and starfish, which now contain worrying levels of pharmaceuticals and heavy metals.

City officials maintain that modelling shows no guideline breaches on the shoreline, but recent reports admitted multiple exceedances in suspended solids, arsenic, and zinc.


It’s Also a Sanitation Inequality Issue

At the heart of this crisis lies a deeper challenge: infrastructure inequality. Informal settlements along river catchments often lack proper waste removal services. With over 170,000 households across 526 informal settlements, many residents are still forced to use buckets due to inaccessible toilet placement.

Dr. Sumaya Clarke from UWC sums it up: “The city is doing what it can. I’m not sure if it’s enough.”


Why It Matters

  • Public health is at risk. Enterococci and E. coli exposure can cause serious infections.
  • Tourism is affected. Signs warning “swim at your own risk” don’t pair well with holiday plans.
  • Biodiversity suffers. Estuaries and ocean ecosystems are under increasing pressure.

What Now?

The MAP Forum calls for:

  • Stronger partnerships between citizen science groups and city officials.
  • Independent water testing and more transparent data.
  • Equitable investment in sanitation infrastructure across all communities.
  • Urgent upgrades to wastewater treatment and outfall management.

Cape Town’s water crisis is a microcosm of South Africa’s wider environmental challenges—where climate, infrastructure, inequality, and accountability all converge.

If we’re to protect our oceans and waterways, we must rethink the stink—and act.
